Before submitting this form, please verify that you meet the criteria listed below. Submissions that do meet these requirements will be rejected.

90 days must have passed since your last interview with the apprenticeship committee.
You must have either gained at least 450 hours of documentable work experience in the electrical industry or have completed two trade related classes since your last interview.
Documentation must be submitted for either condition.

Once your request has been received, it will be evaluated and you will be notified of the result. If a re-interview is granted, the score obtained as a result of that interview will supersede your previous score.
